Your guide to getting reimbursed for medical supplies

We're excited to announce that by August 2026, QG MED will begin accepting insurance and Medicare for eligible medical supplies. Stay tuned — we'll notify you as soon as direct billing is available.

At QG MED, we're committed to making medical supplies accessible and affordable. In the meantime, many of our customers can obtain reimbursements by submitting their own Medicare claim using the steps below.

Submitting Your Own Medicare Claim — Form CMS-1490S

To submit your claim directly to Medicare, use the Medicare Claim Form CMS-1490S (Patient's Request for Medical Payment). This form allows patients to request reimbursement when their supplier is not enrolled with Medicare or is unable to file on their behalf.

Obtain a Prescription

Contact your healthcare provider and request a prescription for the medical supplies or equipment you need. Make sure the prescription includes specific item details and medical necessity.

Place Your Order at QG MED

Shop at QG MED and purchase your required medical supplies. Save your itemized receipt and order confirmation — you'll need this for your claim.

Complete Form CMS-1490S

Download and complete the Medicare Claim Form CMS-1490S. On the form, check the reason box: "The provider or supplier is not enrolled with Medicare." Attach your itemized receipt, prescription, and any other required documentation.

Mail to Your Medicare Administrative Contractor

Send the completed form and documentation to your Medicare Administrative Contractor (MAC) based on your state. The mailing addresses are included in the CMS-1490S PDF. Allow at least 60 days for Medicare to process your request.

Receive Your Reimbursement

Medicare will review your claim and pay you directly for covered items. Reimbursement timing varies but typically takes 4–8 weeks after receipt.

Tips for a Successful Claim

  • •Always verify coverage and eligibility with your insurance provider before purchasing
  • •Keep copies of all documentation for your records
  • •Submit claims promptly — most insurance plans have time limits for reimbursement requests
  • •Make a copy of your completed claim submission before mailing
  • •Medicare will not process claims for diabetic test strips, Part B drugs, or items under the DMEPOS Competitive Bidding program via this form
